New Patient Registration

New patients should contact us by phone to arrange registration or by completing and signing a registration form which can be downloaded  from  Once completed and signed this can be emailed to the practice or posted through the letterbox at the practice.  If you have used the online process to register we will either post or email out a new patient questionnaire to complete. 

On registering you may be invited to have a new patient appointment to record routine clinical information about yourself and for any necessary examinations to be carried out. The examination will normally take place at the surgery but in exceptional circumstances if your doctor considers it necessary this can be carried out in your home.

Please note you are still registered with your previous GP until your new registration has been accepted.  Please ensure you contact your previous GP to obtain enough medication to last until you have registered with the Practice.

When you are accepted on to the Practice list you will not be allocated to a specific doctor. However, you have the right to express a preference to receive services from a particular clinician and to have this recorded in writing at the Practice. We will endeavour to comply with such a request but this may not always be possible.

This Practice operates a zero tolerance policy towards violence or abuse to staff.   

Leaving the practice

If you move away from the practice area you will have to register with a practice in your new area as soon as possible either by contacting the new practice directly or by telephoning Practitioner Services (0131 275 7071 or 7073)

Removal from the practice 

In certain circumstances the practice will request a patient’s removal from the list. This is a rare occurrence and only happens when the patient/doctor relationship has broken down. This can happen when a patient is abusive or violent towards practice staff or Doctors or if a patient is misusing the practice’s services or fails to attend multiple appointments.

A letter informing the patient of the removal and the reason for this will be sent to the patient and also to Practitioner Services. The practice has to give 7 days notice of a removal and in that time the patient should either register with a different practice or contact Practitioner Services (0845 300 1216) to be assigned to a different practice.
